What is Community Case Management?
Community Case Management is a service that helps you connect with support in your local community. We work with you to solve problems that may delay your ability to feel better or reach your goals.
Eligibility
Services are available to Prince George's County residents who meet the following criteria:
- Ages 18 and older
- Currently in treatment or need help connecting to Behavioral Health services

Your Community Case Manager is your dedicated partner in navigating and coordinating the essential services you need.

- Inpatient and Outpatient Mental Health Treatment
- Inpatient and Outpatient Substance Use Treatment
- Peer Support
- Anger Management
- Batterer’s Intervention Program
- Health Insurance and other benefits
- Identifying documents
- Employment and Education
- Transportation
- Housing
