The following Health permit and license applications must be submitted in the new Momentum system.
Agricultural Well Certification
Category Name: Environmental Engineering
Description: Application for the use of an existing water supply system to be maintained for agricultural use. The inspection fee does not include any costs for water sample analysis.
Open Land Burning Permits
Category Name: Environmental Engineering
Description: Application for open fire (Burn) permit for recreational purposes such as campfires, bonfires, fire training, or for fires set in the course of agricultural operations in growing crops or raising fowl or animals or in accepted forestry practices.
Plat Reviews
Category Name: Environmental Engineering
Description: Application for Health Department review of the division (by plat or deed) of a single tract, tracts, or other parcels of land, or a part thereof, into two (2) or more lots, for the purpose, whether immediate or future, of sale or of building development, (including changes in street lines or lot lines).
Bay Restoration Funds
Category Name: Environmental Engineering
Description: Application for grant funds from the State’s Chesapeake Bay Restoration Fund for the installation of Best Available Technology (BAT) nitrogen-reducing septic tanks or connection to the public sewer.
Health Hazard
Category Name: Environmental Engineering
Description: Application in response to a failing or potentially failing private drinking water well or sewage disposal system.
Prospective Pool Inspections
Category Name: Environmental Health
Description: Application for prospective inspections, consultations, and equipment evaluations at a public swimming pool, public wade pool, or public spa.
Health Survey Letter
Category Name: Environmental Engineering
Description: Application for Health Department review of an MSDE facility, foster care or adoptive care home.
Work Order
Category Name: Environmental Engineering
Description: Application for the authorization to perform a minor repair of a sewage disposal system. A repair includes the replacement of any defective part or appurtenance of a sewage disposal system that does not alter the size or configuration of the system, and which is intended to correct a malfunction or to improve the efficiency of operation of the system.
