COVID-19 Update

The health and safety of our residents are our highest priorities. With ongoing concerns about the Coronavirus (COVID-19) and recommendations from state health authorities to limit gatherings of people, we have made the decision to transition from in-person programming to offering sessions online. Classes will meet virtually until COVID-19 restrictions are lifted. Once restrictions are lifted, all online sessions will transition back to in-person meetings.

The Living Well Chronic Disease Self-Management Program is a self-management class for anyone with a chronic condition (arthritis, diabetes, heart disease, hypertension), or any other chronic condition that requires ongoing medication. Classes meet online once a week for 2.5 hours and cover a variety of topics that support developing a healthier lifestyle and how to be a partner with your health care team. Registration is required.

Who Should Register? 

If you have a chronic condition like arthritis, diabetes, heart disease, or any other chronic conditions that require ongoing medication, you can take charge of your health and live well! Registrants should be 18 years and over and have been diagnosed with a chronic condition by a health care professional. Caregivers and/or relatives are welcomed to attend.

2022 Virtual Class Schedule

Session: February 17 - March 31, 2022 (Thursdays)

Class Time: 3:30 pm - 6:00 pm

Class Dates: 2/17 (*orientation session) , 2/24, 3/3, 3/10, 3/17, 3/24, 3/31

Location: Online. The link to the online program will be emailed to registered participants prior to the start of the program.
