The Prince George's County Health Department Vital Records office issues certified copies of Maryland death certificates by appointment only for individuals who passed away in the state after January 1, 2015

  • Tuesdays - 8:00 am to 3:30 pm
  • Wednesdays CLOSED
  • Thursdays - 8:00 am to 3:30 pm
  • Fridays- 8:00 am to 3:30 pm

**Our office is closed daily for lunch from 12:00 pm - 1:00 pm **

How to Get a Maryland Death Certificate

Requests for certified Maryland death certificates may be made at the Cheverly location for deaths that occurred in Maryland after January 1, 2015.

Who Can Request a Copy

You may obtain a Maryland Death Certificate if you are:

  • A funeral director
  • A surviving relative of the deceased
  • A beneficiary of the deceased
  • An authorized representative of the deceased

Fees & Rates

There is a $25 fee for the first certified death certificate and $20 for each additional certificate purchased on the same day. Cash, money orders, and credit or debit cards (Visa, Discover, MasterCard) are accepted as payment.

  • A $2.50 service fee applies to credit/debit card transactions.
  • Check or money order should be made payable to Prince George’s County Health Department
