IMPORTANT HEALTH COVERAGE TAX DOCUMENTS

In accordance with IRS guidance under Sections 6055 and 6056, our organization has chosen to furnish Forms 1095-C upon request rather than mailing them automatically. These forms provide information about your health coverage for tax filing purposes.

How to Request Your Form 1095-C

If you would like to receive a copy of your 1095-C, you may request using any of the methods below:

Download: Visit pgcgov.bswift.com and log in with your username and password. Your username is the first initial of your first name + full last name. If needed, please use the password reset option to access your account. On the home page, click the “Get My Form” button. You will see your form under “Uploaded Documents”. Simply click the “View” button to view or download.

When submitting your request, please include:

· Your full name

· Your employer’s name

· Current mailing address
