Project Home (C.A.R.E. Program)

A Safe, Supportive, and Family-Like Environment

The Certified Adult Residential Environment program (also known as Project Home) is a voluntary supportive housing program in Prince George’s County. We match adults with disabilities who cannot live completely on their own with certified local families who open up their homes to provide long-term, safe, and loving care.

Please Note: Project Home utilizes an Adult Foster Family Model. It is designed for stable, long-term housing and is not an emergency housing or shelter program.

For Prospective Residents

Who We Serve

Project Home assists Prince George’s County residents aged 18 and older who need a safe place to live but do not require round-the-clock nursing home or institutional care.

Our residents are adults who:

  • Need a protected housing environment and cannot live fully independently.
  • Are mobile (or can safely transfer themselves in and out of a wheelchair without assistance).
  • May live with medical conditions (like diabetes, heart conditions, arthritis, or HIV), mental health diagnoses (such as schizophrenia, depression, bipolar disorder, or anxiety), or age-related disabilities.

What is Included?

Residents receive dedicated, personalized support from their host family, including:

  • Three nutritious meals a day plus snacks.
  • Assistance with personal care, dressing, budgeting, and shopping.
  • Reminders and coaching for taking daily medications.
  • Transportation and scheduling help for medical and psychiatric appointments.

For Prospective Care Providers

Open Your Heart and Home

If you love helping others, you can become a certified Project Home provider. Providers can host a maximum of four residents in their home, giving them a true family-like setting.

Provider & Home Requirements

To ensure the safety of our residents, all host families must meet strict guidelines:

  • Background Checks: Anyone in the home over age 18 must pass a criminal background and agency clearance.
  • Backup Support: You must have at least one reliable backup person who can step in during emergencies or in your absence.
  • Training: Free mandatory training is provided, covering CPR, First Aid, medication coaching, and emergency preparedness.
  • Home Safety: Your home must feature working smoke detectors on every floor, fire extinguishers, grab rails in bathrooms/stairways, a landline phone, and a designated common area for relaxation.
  • Resident Bedrooms: Max of 2 residents per room, with minimum space requirements (70 sq. ft. for singles, 120 sq. ft. for doubles) and essential furniture provided.

How it Works: The Matching Process

We want every placement to feel like a perfect fit. Our process includes:

  1. Assessment: A Coordinator and a Registered Nurse review the applicant's eligibility.
  2. Introduction: We suggest a certified provider based on the resident's specific needs and wishes.
  3. Visits: We arrange an initial meet-and-greet, followed by a paid overnight trial visit to ensure everyone gets along.
  4. Moving In: Once everyone agrees, a customized Service Agreement is created, and a move-in date is set.

Cost & Payment

Residents agree to pay for their housing via personal resources or public assistance. Eligible residents will receive assistance applying for Public Assistance to Adults (PAA) and Medical Assistance to help cover costs. Case management services are always provided regardless of income.
