Subpoenas for medical records

Because the Technology & Information Services Office is not one of the parties that needs notice on the issuance date, subpoenas can be sent along with written assurance and other supportive documentation after the 30 day notice period is over. Other subpoenas may be sent to TISO to be forwarded to other offices for fulfillment.

MPIA/PIA/FOIA

Information can be requested on any subject so long as it does not contain Personally Identifying Information (PII), which is protected under the Health Insurance Portability Accountability Act (HIPAA). For instance, if statistics are being gathered on medical response data, medical reports can be released so long as they do not contain any PII and cannot be directly tied to an individual.

For medical record inquiries be sure to include:

•Patient Name. Please indicate if transported as a John/Jane Doe.
•Date of birth / SSN / gender.
•Date of incident.
• Time of incident.
•Location of Incident. Include main street & cross street if it’s an accident.
•Self-addressed and stamped envelope if requesting hard copies.
• Contact information such as a phone number and email address to fulfill to.

For fire record inquiries be sure to include:

•Date of incident.
• Time of incident.
•Vehicle Make/Model & Tag Number (if applicable).
•Location of Incident. Include Main Street & Cross Street.
•Self-addressed, stamped envelope if requesting hard copies.
• Contact information such as a phone number and email address to fulfill to.
