To improve the quality and accessibility of healthcare to the citizens of Prince George’s County through efficient, innovative, and integrated multi-agency means.

To ensure that all citizens of Prince George’s County have coordinated access to a health care system that meets their personal needs and to enhance individual health outcomes.

 

Our Mobile Integrated Healthcare (MIH) Team consists of experienced paramedics and a Licensed Clinical Social Worker (LCSW). This combination allows the MIH program to provide a progressive, patient-centered approach for in-home, out-of-hospital care, and resource connection that will assist with linking enrolled clients to local organizations and unexplored medical/social programs that will meet their needs.

The paramedic, as part of the MIH team, is not meant to act in an expanded scope of practice, however, in an expanded role in the health care system. The role of the MIH paramedic is that of a patient-centered, all-inclusive care model versus an acute care, incident response centered model. 

When a citizen has been identified as a candidate for MIH services, an appointment is scheduled for an in-person visit via telephone or virtual platform. Upon arrival at the residence, a paramedic from the MIH team will perform a full home safety assessment and obtain baseline vital signs on the patient, to include:

  • Blood Pressure
  • Heart Rate
  • Blood Glucose check
  • 3 Lead
  • 12 Lead
  • Oxygen Saturation
  • End Tidal Capnography (when appropriate) 
  • Lung Sounds

Other services that may be provided include:

  • Home safety assessment
  • Fall risk assessment
  • Smoke detector and CO detector check for age and batteries
  • Social support evaluation
  • Medication review and education
  • Condition review and education
  • Physical health assessment
  • Mental health assessment
  • Linkage to community resources as needed
  • Linkage to specialty services including mental health and substance abuse
