Getting Vaccinated
Each COVID-19 vaccine authorized for public use by the U.S. Food and Drug Administration and the Centers for Disease Control and Prevention is available for eligible individuals in Prince George’s County.
The Health Department offers COVID-19 vaccinations at two of its clinics BY APPOINTMENT ONLY on a limited basis for eligible County residents and County employees. Department clinics focus on providing clinical services to residents who are underinsured, uninsured, and those insured under Medical Assistance.
The department’s mobile vaccination units are typically reserved for homebound residents and other high-risk residents who require assistance in getting access to the vaccine. Individuals who are interested in our mobile vaccination units should call 311 and press # for assistance.

Federally Qualified Health Centers (FQHCs)
Federally Qualified Health Centers (FQHCs) in Prince George’s County also offer COVID-19 vaccinations. FQHCs are community-based, federally funded health care providers offering primary care services in underserved areas. Locate an FQHC near you.
Required Consent Forms
Individuals younger than 18 receiving vaccinations must have a parent or legal guardian complete the appropriate consent form and bring it to the clinic.

Request a Vaccination Card
There are two options to request a vaccination card.
Mail:
- email COVID19_VaxCard@co.pg.md.us to request a copy of your CDC vaccination card by mail.
Online:
- For an immediate copy of your record, register at https://md.myir.net/rorl?next=/ to download your vaccination certificate.
