ON THE ROAD Diabetes Outreach Program
LARGO, MD --As healthcare leaders in Prince George's County, both the Prince George's County Health Department (PGCHD) and Doctors Community Hospital (DCH) are committed to empowering community members to improve the health and quality of their lives. In April of 2013, the PGCHD in collaboration with DCH's Joslin Diabetes Center launched the "On the Road" Diabetes Outreach Program to raise awareness about the importance of diabetes screenings, management and prevention.  To celebrate the participants’ achievements, there will be a holiday event on Tuesday, December 3, 2013 from 4:30pm to 6:30pm. at the Prince George’s Sports & Learning Complex on 8001 Sheriff Road, Town Hall A/B in Landover, Maryland 20785.
The theme is “Taking Knowledge to Action.” In addition to cooking demonstrations, holiday dining tips, fun fitness activities and exciting gifts, attendees will be inspired by testimonials from others who are successfully managing their diabetes. “This is a community based approach that brings diabetes education directly to the residents that are impacted the most,” said Pamela B. Creekmur, Health Officer of Prince George’s County Health Department. “This partnership with the renowned Joslin Diabetes Center at Doctors Community Hospital allows us to serve our residents where they live, work and play.”
To date, twelve (12) community sessions have been conducted, which consisted of 7 initial and 5 corresponding follow up classes at convenient locations in Prince George's County for residents, including community centers, faith-based organizations, health clinics, and other localities.  During the two hour sessions, people with diabetes learned how to make lifestyle changes that can minimize the associated health risks.  Also, their families, friends and caregivers were invited to learn more about the management of this disease.
