YOU MUST APPEAR IN PERSON TO FILE A COMPLAINT
During your appointment, you will discuss the issues surrounding your complaint. The Investigator will consult local and federal guidelines to determine if the complaint can be processed by our office. Please make every effort to bring the following on the day of your appointment:
●Valid Picture ID, (preferably a Drivers License);
●Name(s) and address of the company/establishment and/or person that you are filing your complaint against along with information for a contact person, (e.g. an immediate supervisor if you are filing an employment discrimination complaint);
●Name(s), address and telephone numbers of any potential witnesses that you would like the Investigator to interview during the course of the investigation;
●Supporting documentation that you believe will assist you during the complaint process and subsequent investigation. |
