These programs serve all vulnerable adults over the age of 18.
- Adult Protective Services
- This is an investigative service directed toward preventing neglect,
self-neglect, abuse and exploitation of vulnerable adults over the
age of 18. Here are some Vulnerable Adult Indicators.
- Social Services to Adults
- This is the core voluntary service program for persons over age 18.
It focuses on assisting disabled and elderly adults to remain independent
and live in their community for as long as possible.
- Project Home/Certified Adult Residential Environment (C.A.R.E.)
- Supportive housing and case management services are offered to chronically mentally ill adults. There are specific diagnoses requirements to receive services under this program.
- Senior Care
- Purchasing "long- term care" services for frail persons over 65 years old who are at risk of institutionalization is the focus of this program. This program is a collaborative effort between the Health Department, the Department of Social Services and the Office on Aging.
- In-Home Aide Services
- This service provides assistance with activities of daily living for clients who either live alone or with their families. Clients include children, families disabled adults and the elderly. Sliding fee scale is used.
- Long Term Care Medical Assistance
- This unit serves adults who are in need of 24 hour skilled/ supervised care in a nursing facility by determining eligibility for medical assistance. This program works with over 150 nursing homes.
- Respite Care
- This service provides respite care services to caregivers who are caring for a physically, mentally or emotionally disabled loved one.
Other Adult Services Programs
- Adult Foster Care providing foster care services to adults who have need for housing and case management services.
- Adult Guardianship Program - By statute, adults who are determined to be legally incompetent between the ages of 18 and 64 are appointed a legal guardian by the courts. If they have no-one concerned individual, willing to assume guardianship, the Director of the Department of Social Services is appointed as guardian and the Department assumes responsibility to ensure the ward's well-being.
