Monitoring Care to the Elderly at Home

Medical Assistance Personal Care Program

The Medical Assistance Personal Care Program is available for frail elderly individuals with chronic diseases or developmentally disabled persons who need assistance with daily living activities. A community health nurse assesses the patients' needs and monitors care given by a provider in the home. To be eligible for this service, the patient must have a Medical Assistance number.
