How to Get a Maryland Death Certificate
For Certificates Filed with the Prince George's County Health Department Vital Records Office
Requests for certified Maryland Death Certificates may now be made on a walk-in basis in Prince George's County at the Office of Vital Records, Prince George's County Health Department in Landover, Maryland (location and hours are listed at below) within 30 days of the date of death.
NOTE: The Certificate may be available if originally filed in Prince Georges County. Although the death occurred in Prince George's County, it may not be on file with the Prince George's County Health Department Vital Records Office.
Please read the following information or call before going to the Prince George's County Health Department Vital Records Office to expedite a request.
You may obtain a Maryland Death Certificate if you are:
- A funeral director
- An immediate relative of the deceased
- A beneficiary of the deceased
- A person of tangible interest
There is a $20.00 fee for each certified Death Certificate copy. Cash, check or money order accepted as payment.
Prior to coming to the Medical and Vital Records office, print the Request For a Copy of A Death Certificate Form. (PDF)
Bring the completed form and $20.00 payment during office hours at the location listed below.
For Certificates Not Filed with the Prince George's County Health Department Vital Records Office, or
For Requests Made 31 days after the Date of Death
